l\u2019ovaire est le septi\u00e8me cancer le plus r\u00e9pandu chez les femmes, avec une incidence plus \u00e9lev\u00e9e dans les pays d\u00e9velopp\u00e9s.<br \/>\nLe risque au cours de la vie d&#8217;une femme de d\u00e9velopper un cancer de l&#8217;ovaire est estim\u00e9 \u00e0 moins de 2 %, la plupart des cas \u00e9tant diagnostiqu\u00e9s entre 50 et 59 ans.<\/p>\n<p>Malheureusement, le diagnostic est g\u00e9n\u00e9ralement pos\u00e9 \u00e0 un stade avanc\u00e9, car les sympt\u00f4mes tardent \u00e0 appara\u00eetre. Cependant, \u00e0 un stade pr\u00e9coce, la survie avec un traitement appropri\u00e9 d\u00e9passe 90 % \u00e0 cinq ans.<\/p>\n<h2><strong>Stades<\/strong><\/h2>\n<p>Au stade 1, le cancer est limit\u00e9 \u00e0 1 ou 2 ovaires ou trompes de Fallope et ne s&#8217;est pas propag\u00e9 \u00e0 d&#8217;autres organes.<\/p>\n<p>Au stade 2, le cancer s&#8217;est propag\u00e9 \u00e0 d&#8217;autres organes du bassin en plus des ovaires et des trompes de Fallope.<\/p>\n<p>Au stade 3, le cancer, outre les ovaires et les trompes de Fallope, s&#8217;est propag\u00e9 \u00e0 d&#8217;autres organes, en dehors du bassin, et\/ou aux ganglions lymphatiques, et\/ou \u00e0 la surface du foie.<\/p>\n<p>Au stade 4, le cancer s&#8217;est propag\u00e9 \u00e0 des parties \u00e9loign\u00e9es du corps, comme l&#8217;int\u00e9rieur du foie et les poumons.<\/p>\n<h2><strong>Esp\u00e9rance de vie<\/strong><\/h2>\n<p>Au stade 1, le pronostic est d&#8217;environ 90 % \u00e0 5 ans apr\u00e8s le diagnostic.<\/p>\n<p>Au stade 2, \u00e0 71%.<\/p>\n<p>Au stade 3, \u00e0 41%.<\/p>\n<p>Au stade 4, \u00e0 20%.<\/p>\n<h2><strong>Sympt\u00f4mes<\/strong><\/h2>\n<p>Celles-ci sont non sp\u00e9cifiques et peuvent passer longtemps inaper\u00e7ues aupr\u00e8s de la femme. Sensation de pl\u00e9nitude et de sati\u00e9t\u00e9 abdominale, flatulences (gonflement), douleurs abdominales, fr\u00e9quence, perte d&#8217;app\u00e9tit, en font partie.<\/p>\n<p>Le plus souvent, le diagnostic est pos\u00e9 fortuitement, apr\u00e8s un examen ou une \u00e9chographie gyn\u00e9cologique et la d\u00e9couverte d&#8217;une tumeur au niveau de l&#8217;abdomen.<\/p>\n<h2><strong>Diagnostic et examens<\/strong><\/h2>\n<p>Nous ne disposons toujours pas d&#8217;une m\u00e9thode fiable de pr\u00e9vention du cancer de l&#8217;ovaire qui soit appliqu\u00e9e syst\u00e9matiquement et \u00e0 grande \u00e9chelle, comme cela se fait par exemple avec le test Pap et le cancer du col de l&#8217;ut\u00e9rus (\u00e0 noter que le test Pap est un examen qui ne couvre pas les ovaires, ce qui signifie que m\u00eame si le test Pap est normal, il n&#8217;exclut pas l&#8217;existence d&#8217;une malignit\u00e9 ovarienne).<\/p>\n<p>Les seuls tests qui peuvent nous donner des indications (qui sont appliqu\u00e9s en cas de suspicion de malignit\u00e9) sont l&#8217;\u00e9chographie et la mesure de CA-125 dans le sang.<\/p>\n<p>Cependant, des \u00e9tudes montrent que ces tests ne r\u00e9duisent pas les taux de mortalit\u00e9 en g\u00e9n\u00e9ral, mais peuvent avoir un avantage dans la d\u00e9tection des femmes pr\u00e9sentant un risque \u00e9lev\u00e9 de cancer de l&#8217;ovaire, comme l&#8217;h\u00e9r\u00e9dit\u00e9. De mani\u00e8re caract\u00e9ristique, nous rapportons que le CA-125 est \u00e9lev\u00e9 dans le sang chez 50 % des femmes pr\u00e9sentant une tumeur maligne \u00e0 un stade pr\u00e9coce, mais il peut \u00e9galement \u00eatre \u00e9lev\u00e9 dans d&#8217;autres maladies gyn\u00e9cologiques <strong>b\u00e9nignes<\/strong> telles que <a href=\"https:\/\/gynequipe.gr\/fr\/femme\/chirurgie\/endomitriosi-2\/\">l&#8217;endom\u00e9triose<\/a>.<\/p>\n<p>Par la suite et en cas de suspicion de cancer de l&#8217;ovaire, d&#8217;autres examens peuvent \u00eatre effectu\u00e9s, tels que l&#8217;imagerie par r\u00e9sonance magn\u00e9tique, la tomodensitom\u00e9trie et le pet-scan.<\/p>\n<h2><strong>Facteurs de risque<\/strong><\/h2>\n<ul>\n<li>Ant\u00e9c\u00e9dent familial<\/li>\n<li>L&#8217;h\u00e9r\u00e9dit\u00e9, touche 5 \u00e0 10 % de tous les cancers de l&#8217;ovaire (syndrome de Lynch 2, oncog\u00e8nes BRCA1 et 2)<\/li>\n<li>\u00c2ge croissant<\/li>\n<li>Infertilit\u00e9<\/li>\n<\/ul>\n<h2><strong>Facteurs de protection<\/strong><\/h2>\n<ul>\n<li>Grossesse<\/li>\n<li><a href=\"https:\/\/gynequipe.gr\/fr\/femme\/gynecologie\/la-pilule-du-lendemain\/\">Pilule contraceptive<\/a><\/li>\n<li>Allaitement maternel<\/li>\n<li><a href=\"https:\/\/gynequipe.gr\/fr\/femme\/chirurgie\/plastie-tubaire\/\">Ligature des trompes<\/a><\/li>\n<li><a href=\"https:\/\/gynequipe.gr\/fr\/femme\/chirurgie\/hysterectomie\/\">Hyst\u00e9rectomie<\/a> (ablation de l&#8217;ut\u00e9rus)<\/li>\n<\/ul>\n<h2><strong>H\u00e9r\u00e9dit\u00e9<\/strong><\/h2>\n<p>Comme nous l\u2019avons mentionn\u00e9 ci-dessus, l\u2019h\u00e9r\u00e9dit\u00e9 est un facteur de risque important pour le d\u00e9veloppement du cancer de l\u2019ovaire.<br \/>\nCes derni\u00e8res ann\u00e9es, la publicit\u00e9 intense a donn\u00e9 l&#8217;impression qu&#8217;avec des tests g\u00e9n\u00e9tiques appropri\u00e9s, nous pouvons pr\u00e9dire tous les cas de cancer de l&#8217;ovaire. Mais l\u2019h\u00e9r\u00e9dit\u00e9 n\u2019est responsable que de 5 \u00e0 10 % de tous les cancers de l\u2019ovaire, alors que ces tests sont complexes, longs, et tr\u00e8s co\u00fbteux.<br \/>\nPour cette raison, c\u2019est l\u2019histoire medicale qui va d\u00e9terminer le d\u00e9roulement de ces examens sur la femme mais aussi sur son environnement imm\u00e9diat.<\/p>\n<p>Le probl\u00e8me le plus fr\u00e9quent se retrouve dans la population pr\u00e9sentant une mutation des g\u00e8nes BRCA-1 et BRCA-2, un pourcentage correspondant \u00e0 0,25% de la population europ\u00e9enne g\u00e9n\u00e9rale.<br \/>\n<strong>Les tests g\u00e9n\u00e9tiques sont g\u00e9n\u00e9ralement recommand\u00e9s et couverts par EOPYY dans :<\/strong><\/p>\n<ul>\n<li>Toutes les femmes ayant re\u00e7u un diagnostic de cancer \u00e9pith\u00e9lial de l&#8217;ovaire, quels que soient leur \u00e2ge et leurs ant\u00e9c\u00e9dents familiaux.<\/li>\n<li>Toutes les femmes ayant re\u00e7u un diagnostic de cancer du sein avant l&#8217;\u00e2ge de 45 ans.<\/li>\n<li>Toutes les femmes ayant re\u00e7u un diagnostic de Ca du sein \u00e0 tout \u00e2ge et qui ont au moins deux (2) cas du m\u00eame c\u00f4t\u00e9 de la famille<\/li>\n<li>avec un diagnostic approfondi de Ca du sein, des ovaires, du pancr\u00e9as ou de la prostate avec un score de Cleason sup\u00e9rieur ou \u00e9gal. \u00e0 7.<\/li>\n<li>Chez les femmes dont la famille poss\u00e8de une mutation connue des g\u00e8nes BRCA 1\/2.<\/li>\n<li>Si une mutation est d\u00e9tect\u00e9e, les autres membres de la famille doivent \u00e9galement \u00eatre test\u00e9s, sur la base d&#8217;un conseil g\u00e9n\u00e9tique.<\/li>\n<\/ul>\n<p><strong>S&#8217;il n&#8217;est pas possible de v\u00e9rifier le membre concern\u00e9, la v\u00e9rification suivante peut \u00eatre effectu\u00e9e si vous avez :<\/strong><\/p>\n<ul>\n<li>Ant\u00e9c\u00e9dents de cancer du sein avant l&#8217;\u00e2ge de 40 ans chez un parent au premier ou au deuxi\u00e8me degr\u00e9<\/li>\n<li>Un parent au premier ou au deuxi\u00e8me degr\u00e9 de toute personne e \u00e2ge, avec un cancer du sein <strong>et<\/strong> des ovaires<\/li>\n<li>Deux cas ou plus de cancer du sein chez des apparent\u00e9s au premier ou au deuxi\u00e8me degr\u00e9, dont un de moins de 50 ans ou bilat\u00e9ral (les deux seins)<\/li>\n<li>Un parent au premier ou deuxi\u00e8me degr\u00e9 atteint d&#8217;un cancer du sein de moins de 50 ans ou bilat\u00e9ral et un parent au premier ou deuxi\u00e8me degr\u00e9 atteint d&#8217;un cancer de l&#8217;ovaire<\/li>\n<li>Trois cas de cancer du sein et de l&#8217;ovaire (dont au moins un de l&#8217;ovaire) chez des apparent\u00e9s au premier et au deuxi\u00e8me degr\u00e9<\/li>\n<li>Deux cas de cancer de l&#8217;ovaire chez des apparent\u00e9s au premier et au deuxi\u00e8me degr\u00e9<\/li>\n<li>Un cas de cancer du sein chez un parent de sexe masculin au premier ou au deuxi\u00e8me degr\u00e9, si un autre parent au premier ou au deuxi\u00e8me degr\u00e9 (homme ou femme) est atteint d&#8217;un cancer du sein ou des ovaires.<\/li>\n<li>Vous \u00eates d&#8217;origine juive (ashk\u00e9naze) et un parent a d\u00e9velopp\u00e9 un cancer de l&#8217;ovaire ou du sein.<\/li>\n<\/ul>\n<p>Plus pr\u00e9cis\u00e9ment, les porteurs de la mutation <strong>BRCA-1<\/strong> pr\u00e9sentent un risque cumul\u00e9 jusqu\u2019\u00e0 70 ans :<\/p>\n<ul>\n<li>60 % pour le cancer du sein<\/li>\n<li>59 % pour le cancer de l&#8217;ovaire<\/li>\n<li>83 % pour le cancer du sein controlat\u00e9ral<\/li>\n<\/ul>\n<p>Pour les porteurs de la mutation <strong>BRCA-2<\/strong>, les taux de risque cumul\u00e9s jusqu\u2019\u00e0 70 ans sont :<\/p>\n<ul>\n<li>55 % pour le cancer du sein<\/li>\n<li>16,5% pour le cancer de l&#8217;ovaire<\/li>\n<li>62 % pour le cancer du sein controlat\u00e9ral<\/li>\n<\/ul>\n<p>Par cons\u00e9quent, le suivi recommand\u00e9 pour les femmes porteuses de ces mutations, en ce qui concerne les ovaires, est deux fois par an un examen clinique par le gyn\u00e9cologue, une \u00e9chographie et une mesure du CA-125 dans le sang, \u00e0 partir de la \u00e2ge de 30 ans ou 5 ans avec 10 ans plus t\u00f4t que l&#8217;\u00e2ge d&#8217;apparition du cancer de l&#8217;ovaire chez un membre de la famille. L&#8217;ablation chirurgicale des ovaires est \u00e9galement recommand\u00e9e apr\u00e8s l&#8217;\u00e2ge de 35 \u00e0 40 ans ou apr\u00e8s la fin du planning familial.<\/p>\n<p>Moins courant dans la population g\u00e9n\u00e9rale est le syndrome de Lynch, un syndrome h\u00e9r\u00e9ditaire qui associe diff\u00e9rents types de cancer \u00e0 un \u00e2ge plus jeune, comme le cancer du c\u00f4lon, de l&#8217;endom\u00e8tre, des ovaires et du syst\u00e8me urinaire.<\/p>\n<h2><strong>Traitement<\/strong><\/h2>\n<p>Le traitement du cancer de l&#8217;ovaire est principalement chirurgical et c&#8217;est ce qui d\u00e9termine l&#8217;esp\u00e9rance de vie, plus il est pr\u00e9coce et \u00e9tendu, mieux c&#8217;est.<br \/>\nElle consiste en l\u2019ablation totale de l\u2019ut\u00e9rus, des ovaires et des trompes de Fallope, de l\u2019ut\u00e9rus ainsi que de toute autre l\u00e9sion visible m\u00e9tastatique, voire d\u2019une partie de l\u2019intestin si n\u00e9cessaire.<br \/>\nIl s\u2019agit d\u2019op\u00e9rations majeures qui se font g\u00e9n\u00e9ralement par m\u00e9thode ouverte (incision m\u00e9diane).<br \/>\nApr\u00e8s l&#8217;intervention chirurgicale et en fonction des r\u00e9sultats de l&#8217;histologie, il sera d\u00e9cid\u00e9 si le patient devra recevoir un r\u00e9gime de chimioth\u00e9rapie.<br \/>\nAux stades avanc\u00e9s de la maladie, un sch\u00e9ma de chimioth\u00e9rapie est pr\u00e9f\u00e9rable avant la chirurgie pour r\u00e9duire la maladie et optimiser la chirurgie.<br \/>\nChez les jeunes femmes en \u00e2ge de procr\u00e9er, il existe la possibilit\u00e9 de pr\u00e9server l&#8217;ut\u00e9rus et, si les conditions et le stade de la maladie le permettent, de parvenir \u00e0 une grossesse.<br \/>\nComme dans tout cas oncologique, le traitement optimal est individualis\u00e9 et r\u00e9sulte d\u2019une \u00e9quipe de m\u00e9decins de diverses sp\u00e9cialit\u00e9s.<\/p>\n<h2><strong>Tumeurs borderline<\/strong><\/h2>\n<p>Une situation particuli\u00e8re concerne les tumeurs borderline de l&#8217;ovaire, qui, comme le mot l&#8217;indique, se situent \u00e0 la fronti\u00e8re entre b\u00e9nignit\u00e9 et malignit\u00e9.<br \/>\nCes tumeurs peuvent aussi appara\u00eetre \u00e0 des \u00e2ges plus jeunes, le diagnostic n&#8217;est pas toujours facile mais en g\u00e9n\u00e9ral elles ont un tr\u00e8s bon pronostic avec
