bt_bb_bottom_spacing_small\" data-bt-override-class=\"null\"><\/div><div  class=\"bt_bb_text\" ><\/p>\n<p>Il s\u2019agit de tumeurs malignes touchant l\u2019int\u00e9rieur du corps de l\u2019ut\u00e9rus, appel\u00e9 endom\u00e8tre.<\/p>\n<h2><strong>Fr\u00e9quence et \u00e2ge<\/strong><\/h2>\n<p>Il s&#8217;agit du cancer gyn\u00e9cologique le plus r\u00e9pandu dans les pays d\u00e9velopp\u00e9s, avec une incidence de 12,9 pour 100 000 femmes et un \u00e2ge moyen au diagnostic de 61 ans. Cependant, cela peut \u00e9galement survenir chez des femmes beaucoup plus jeunes, en \u00e2ge de procr\u00e9er. Dans ces cas, qui sont g\u00e9n\u00e9ralement diagnostiqu\u00e9s \u00e0 un stade pr\u00e9coce et, dans certains cas, le traitement peut \u00eatre conservateur jusqu&#8217;\u00e0 l&#8217;accouchement. Apr\u00e8s l&#8217;accouchement, l&#8217;ablation de l&#8217;ut\u00e9rus est recommand\u00e9e.<\/p>\n<p>Le type le plus courant et celui avec le meilleur pronostic est l\u2019ad\u00e9nocarcinome endom\u00e9trio\u00efde et on le retrouve dans plus de 80 % des cancers de l\u2019endom\u00e8tre.<br \/>\nOutre le cancer de l&#8217;endom\u00e8tre (la paroi interne de l&#8217;ut\u00e9rus), il existe d&#8217;autres cancers de l&#8217;ut\u00e9rus moins courants qui touchent le corps de l&#8217;ut\u00e9rus, comme les sarcomes, et dont la prise en charge est similaire \u00e0 celle du cancer de l&#8217;endom\u00e8tre.<\/p>\n<p>Les facteurs de risque sont une exposition prolong\u00e9e aux \u0153strog\u00e8nes (exog\u00e8nes ou endog\u00e8nes), une am\u00e9norrh\u00e9e chronique (absence de r\u00e8gles), l&#8217;ob\u00e9sit\u00e9, l&#8217;hypertension, le diab\u00e8te (sucre), des premi\u00e8res r\u00e8gles pr\u00e9coces, une m\u00e9nopause tardive, des ant\u00e9c\u00e9dents (syndrome de Lynch 2).<br \/>\nAu contraire, il existe des facteurs qui prot\u00e8gent contre le cancer de l&#8217;endom\u00e8tre, comme les contraceptifs (jusqu&#8217;\u00e0 50 % de r\u00e9duction du risque), les grossesses \u00e0 un \u00e2ge plus avanc\u00e9, la consommation de caf\u00e9 et de th\u00e9 vert, le tabac (les bienfaits du tabac sont bien moins importants que ses effets d\u00e9sastreux sur la sant\u00e9 humaine).<\/p>\n<h2><strong>Sympt\u00f4mes<\/strong><\/h2>\n<p>Les sympt\u00f4mes se manifestent presque toujours par des saignements vaginaux indolores, qui doivent toujours \u00eatre recherch\u00e9s chez les femmes m\u00e9nopaus\u00e9es.<br \/>\nChez les femmes en \u00e2ge de procr\u00e9er, l\u2019investigation doit \u00eatre effectu\u00e9e lorsqu\u2019elles pr\u00e9sentent des saignements les jours non menstruels.<\/p>\n<p>Le premier examen chez le gyn\u00e9cologue sera une \u00e9chographie transvaginale avec laquelle nous aurons une \u00e9valuation de l&#8217;\u00e9tat en mesurant l&#8217;endom\u00e8tre puis, si cela est jug\u00e9 n\u00e9cessaire, une biopsie suivra, g\u00e9n\u00e9ralement avec curetage.<\/p>\n<p>D\u00e9sormais, si le diagnostic est confirm\u00e9 histologiquement, un scanner et\/ou une IRM nous donneront plus d&#8217;informations sur l&#8217;\u00e9tendue de la maladie et le plan chirurgical sera d\u00e9cid\u00e9.<\/p>\n<h2><strong>Traitement<\/strong><\/h2>\n<p>L&#8217;intervention chirurgicale comprend l&#8217;ablation de l&#8217;ut\u00e9rus ainsi que du col de l&#8217;ut\u00e9rus et des ovaires et \u00e9ventuellement des ganglions lymphatiques (en appliquant g\u00e9n\u00e9ralement la technique des ganglions lymphatiques \u00ab sentinelles \u00bb) en fonction du contr\u00f4le histologique et d&#8217;imagerie initial. Dans les premiers stades, l\u2019intervention chirurgicale est r\u00e9alis\u00e9e en toute s\u00e9curit\u00e9 gr\u00e2ce \u00e0 la m\u00e9thode laparoscopique, contribuant ainsi de mani\u00e8re significative au r\u00e9tablissement rapide du patient.<\/p>\n<p>Apr\u00e8s l&#8217;op\u00e9ration, apr\u00e8s avoir fait ce que nous appelons le stadification de la maladie et l&#8217;histologie finale, un comit\u00e9 d&#8217;oncologie est convoqu\u00e9 et il est d\u00e9cid\u00e9, en collaboration avec des oncologues m\u00e9dicaux, des radiologues, des radioth\u00e9rapeutes et des pathologistes, si un traitement ult\u00e9rieur autre que chirurgie est n\u00e9cessaire.
