de l&#8217;ut\u00e9rus est le dixi\u00e8me cancer le plus r\u00e9pandu parmi les cancers f\u00e9minins (9 femmes sur 100 000) et le troisi\u00e8me parmi les organes gyn\u00e9cologiques. \u00c0 l\u2019inverse, dans les pays en d\u00e9veloppement, c\u2019est le deuxi\u00e8me cancer le plus r\u00e9pandu chez les femmes en termes d\u2019incidence (18 pour 100 000) et de mortalit\u00e9 (10 pour 100 000).<\/p>\n<p>Cette diff\u00e9rence est bien entendu due au syst\u00e8me organis\u00e9 de soins et de pr\u00e9vention avec le test Pap qui est appliqu\u00e9 dans les pays d\u00e9velopp\u00e9s. La vaccination contre le HPV (virus du papillome humain) est \u00e9galement une aide importante dans la lutte contre le cancer du col de l&#8217;ut\u00e9rus, puisqu&#8217;il a \u00e9t\u00e9 prouv\u00e9 que 99 % des cancers du col de l&#8217;ut\u00e9rus sont dus \u00e0 ce virus.<br \/>\nUn exemple typique est celui de l\u2019Australie, o\u00f9 les taux de vaccination d\u00e9passent 70 %, ce qui entra\u00eene une r\u00e9duction de 38 % des l\u00e9sions pr\u00e9canc\u00e9reuses du cancer du col de l\u2019ut\u00e9rus.<\/p>\n<p>L&#8217;\u00e2ge moyen du diagnostic est de 48 ans, tandis que les facteurs de risque li\u00e9s \u00e0 la possibilit\u00e9 d&#8217;une infection par le VPH sont r\u00e9sum\u00e9s ci-dessous :<\/p>\n<ul>\n<li>Initiation pr\u00e9matur\u00e9e \u00e0 l&#8217;activit\u00e9 sexuelle<\/li>\n<li>Plusieurs partenaires sexuels<\/li>\n<li>Ant\u00e9c\u00e9dents de maladies sexuellement transmissibles (chlamydia, herp\u00e8s)<\/li>\n<li>Immunosuppression<\/li>\n<li>Fumer<\/li>\n<\/ul>\n<p>Bien que la majorit\u00e9 des cancers du col de l\u2019ut\u00e9rus soient li\u00e9s au VPH, seul un tr\u00e8s faible pourcentage de femmes ayant \u00e9t\u00e9 expos\u00e9es au virus d\u00e9velopperont un cancer. \u00c0 titre indicatif, on estime que jusqu\u2019\u00e0 80 % des femmes sexuellement actives seront infect\u00e9es par le VPH avant l\u2019\u00e2ge de 50 ans. Cependant, une simple infection par le virus ne suffit pas, car il doit appartenir \u00e0 des souches sp\u00e9cifiques (\u00e0 haut risque) dont il a \u00e9t\u00e9 prouv\u00e9 qu&#8217;elles conduisent \u00e0 l&#8217;apparition du cancer du col de l&#8217;ut\u00e9rus.<\/p>\n<p>Il a \u00e9t\u00e9 constat\u00e9 que ces souches sont sup\u00e9rieures \u00e0 15, les types 16 et 18 \u00e9tant responsables de plus de 70 % des cas de cancer du col de l&#8217;ut\u00e9rus. Apr\u00e8s tout, ce sont ces deux types de vaccins qui sont cibl\u00e9s par les vaccins commerciaux.<\/p>\n<p>La plupart des infections par le virus HPV sont transitoires, car le corps lui-m\u00eame d\u00e9veloppe souvent des anticorps et \u00e9limine le virus \u00e0 temps. De plus, la simple pr\u00e9sence du virus dans le col de l\u2019ut\u00e9rus n\u2019est pas suffisante en soi pour le d\u00e9veloppement d\u2019un cancer. Si la pr\u00e9sence du virus persiste dans le temps, il peut alors \u00e9voluer vers une l\u00e9sion pr\u00e9canc\u00e9reuse puis vers un cancer, un processus qui prend en moyenne 15 ans. Aux stades avanc\u00e9s du cancer invasif du col de l&#8217;ut\u00e9rus, des s\u00e9cr\u00e9tions vaginales naus\u00e9abondes, des douleurs pelviennes, des probl\u00e8mes de pression au niveau de la vessie, de l&#8217;uret\u00e8re (hydron\u00e9phrose pouvant m\u00eame entra\u00eener une perte r\u00e9nale) ou encore au niveau de l&#8217;intestin peuvent appara\u00eetre.<br \/>\nEn cas de suspicion de cancer du col de l&#8217;ut\u00e9rus, soit cliniquement par le gyn\u00e9cologue, soit microscopiquement par le test Pap, alors pour le diagnostic final, il sera n\u00e9cessaire de r\u00e9aliser un examen colposcopique par le gyn\u00e9cologue avec biopsies et une excision conique du col de l&#8217;ut\u00e9rus.<br \/>\nA partir de l\u00e0, apr\u00e8s le diagnostic final, est r\u00e9alis\u00e9 le bilan qui comprend l&#8217;examen clinique sous anesth\u00e9sie et certains examens d&#8217;imagerie comme le scanner ou l&#8217;imagerie par r\u00e9sonance magn\u00e9tique, ou encore le Pet-scan, qui nous renseigneront sur le stade de l&#8217;extension.<\/p>\n<h2><strong>Traitement<\/strong><\/h2>\n<p>Bien entendu, le traitement sera proportionnel au stade, avec un meilleur pronostic dans les premiers stades, comme c&#8217;est logique.<br \/>\nChaque cas est unique, dont le traitement prend en compte de nombreux facteurs comme le stade, l&#8217;\u00e2ge, le d\u00e9sir de la femme, l&#8217;\u00e9tat de sant\u00e9 g\u00e9n\u00e9ral, etc.<br \/>\nUn traitement sp\u00e9cial est n\u00e9cessaire pour les femmes qui n&#8217;ont pas accouch\u00e9 et, si les conditions le permettent, nous appliquons un traitement chirurgical conservateur afin qu&#8217;elles puissent imm\u00e9diatement avoir une grossesse.<\/p>\n<p>La chirurgie (excision cono\u00efde, hyst\u00e9rectomie simple, hyst\u00e9rectomie radicale), la radioth\u00e9rapie, la chimioth\u00e9rapie ou leur association sont les traitements habituellement recommand\u00e9s.<\/p>\n<h2><strong>Stades<\/strong><\/h2>\n<p>Stade 1 : Le cancer est confin\u00e9 \u00e0 la r\u00e9gion cervicale avec une esp\u00e9rance de vie \u00e0 5 ans d&#8217;environ 95 %.<\/p>\n<p>Stade 2 : Le cancer s&#8217;est propag\u00e9 au-del\u00e0 du col de l&#8217;ut\u00e9rus mais ne s&#8217;est pas propag\u00e9 \u00e0 la partie inf\u00e9rieure du vagin ou \u00e0 la paroi pelvienne avec une esp\u00e9rance de vie de 5 ans d&#8217;environ 70 %.<\/p>\n<p>Stade 3 : Le cancer s&#8217;est propag\u00e9 \u00e0 la partie inf\u00e9rieure du vagin et\/ou \u00e0 la paroi pelvienne lat\u00e9rale et\/ou provoque une hydron\u00e9phrose et\/ou un dysfonctionnement des reins et\/ou des ganglions lymphatiques avec une esp\u00e9rance de vie \u00e0 5 ans sup\u00e9rieure \u00e0 40 %.<\/p>\n<p>Stade 4 : Le cancer s&#8217;est propag\u00e9 au-del\u00e0 du bassin et a envahi la vessie et\/ou l&#8217;intestin avec une esp\u00e9rance de vie \u00e0 5 ans d&#8217;environ 15 %.<\/p>\n<p>De ce qui pr\u00e9c\u00e8de, il ressort que le cancer du col de l&#8217;ut\u00e9rus est un cancer qui touche <strong>toutes<\/strong> les femmes quel que soit leur \u00e2ge et qui, gr\u00e2ce au <strong>test Pap<\/strong>, peut \u00eatre pr\u00e9venu \u00e0 un stade <strong>pr\u00e9canc\u00e9reux<\/strong> ou m\u00eame \u00e0 un stade pr\u00e9coce, avec d&#8217;excellents taux de gu\u00e9rison et qualit\u00e9 de vie.<br \/>\nC&#8217;est aussi le sens de
