gyn\u00e9cologique le plus fr\u00e9quent apr\u00e8s le cancer de l&#8217;endom\u00e8tre, de l&#8217;ovaire et du col de l&#8217;ut\u00e9rus, repr\u00e9sentant 5 % des cancers g\u00e9nitaux.<\/p>\n<p>On la retrouve principalement chez les femmes m\u00e9nopaus\u00e9es, avec un \u00e2ge moyen de 65 ans, \u00e2ge en constante diminution ces derni\u00e8res ann\u00e9es.<\/p>\n<p>Les facteurs de risque comprennent le tabagisme, les dystrophies vulvaires en g\u00e9n\u00e9ral (par exemple le lichen), les l\u00e9sions pr\u00e9canc\u00e9reuses de la vulve ou du col de l&#8217;ut\u00e9rus, le virus du papillome humain (VPH), divers syndromes d&#8217;immunosuppression et des ant\u00e9c\u00e9dents de cancer du col de l&#8217;ut\u00e9rus.<\/p>\n<p>60 % des cancers de la vulve sont dus au virus HPV, tandis que le pourcentage restant est d\u00fb \u00e0 des pathologies provoquant une inflammation chronique de la vulve. L\u2019implication du VPH semble \u00eatre responsable du fait que l\u2019\u00e2ge moyen d\u2019apparition diminue de plus en plus ces derni\u00e8res ann\u00e9es et notamment chez les femmes fumeuses.<\/p>\n<h2><strong>Sympt\u00f4mes <\/strong><\/h2>\n<p>La plupart des patients pr\u00e9sentent une l\u00e9sion ou un ulc\u00e8re unique sur les l\u00e8vres, le p\u00e9rin\u00e9e ou le clitoris. Les d\u00e9mangeaisons, bien que l&#8217;un des inconforts les plus courants de la r\u00e9gion g\u00e9nitale, peuvent en \u00eatre le premier sympt\u00f4me, en particulier chez les femmes souffrant de lichen scl\u00e9reux. Cependant, de nombreuses femmes ne ressentent aucune g\u00eane et le diagnostic est pos\u00e9 apr\u00e8s un examen al\u00e9atoire chez le gyn\u00e9cologue. En outre, des saignements, des pertes vaginales, des difficult\u00e9s \u00e0 uriner ou la palpation d&#8217;un ganglion lymphatique dans la r\u00e9gion de l&#8217;aine sont des sympt\u00f4mes qui apparaissent g\u00e9n\u00e9ralement \u00e0 un stade avanc\u00e9.<\/p>\n<h2><strong>Stades<\/strong><\/h2>\n<p>Stade 1 : Le cancer est strictement confin\u00e9 \u00e0 la r\u00e9gion vulvaire, avec une esp\u00e9rance de vie d&#8217;environ 86 % \u00e0 5 ans apr\u00e8s le diagnostic.<\/p>\n<p>Stade 2 : Le cancer s&#8217;est propag\u00e9 aux structures adjacentes de la r\u00e9gion vulvaire sans atteinte des ganglions lymphatiques, avec une esp\u00e9rance de vie de 50 % 5 ans apr\u00e8s le diagnostic.<\/p>\n<p>Stade 3 : Le cancer s&#8217;est propag\u00e9 aux structures p\u00e9rin\u00e9ales adjacentes et\/ou \u00e0 l&#8217;atteinte des ganglions lymphatiques, avec une esp\u00e9rance de vie de 40 % 5 ans apr\u00e8s le diagnostic.<\/p>\n<p>Stade 4 : Le cancer s&#8217;est propag\u00e9 au-del\u00e0 du bassin, avec une esp\u00e9rance de vie de 23 % 5 ans apr\u00e8s le diagnostic.<\/p>\n<p>Le diagnostic se fait dans tous les cas par une <strong>biopsie<\/strong> de la zone suspecte car de nombreuses affections b\u00e9nignes peuvent donner l&#8217;impression d&#8217;une malignit\u00e9.<\/p>\n<h2><strong>Traitement<\/strong><\/h2>\n<p>Puis, une fois le diagnostic pos\u00e9, comme pour toute tumeur maligne, le bilan de la maladie s&#8217;ensuit gr\u00e2ce aux m\u00e9thodes d&#8217;imagerie (IRM, Pet Scan, etc.). A partir de l\u00e0, est d\u00e9cid\u00e9 le traitement chirurgical dont l\u2019ampleur d\u00e9pend de l\u2019ampleur des d\u00e9g\u00e2ts, de l\u2019\u00e2ge de la patiente, de sa condition physique et bien s\u00fbr de son envie. Apr\u00e8s l\u2019intervention chirurgicale, une d\u00e9cision sera prise quant \u00e0 savoir si une radioth\u00e9rapie et\/ou une chimioth\u00e9rapie suppl\u00e9mentaire seront n\u00e9cessaires.<\/p>\n<p>\n<\/div><div class=\"bt_bb_separator bt_bb_border_style_none bt_bb_top_spacing_small bt_bb_bottom_spacing_small\"
