femme lorsque l&#8217;ut\u00e9rus, la vessie, le rectum (une partie du gros intestin) ou l&#8217;ensemble se projettent dans le vagin (respectivement hyst\u00e9roc\u00e8le, cystoc\u00e8le, rectoc\u00e8le).<br \/>\nSelon le degr\u00e9 du prolapsus, la projection peut \u00eatre limit\u00e9e \u00e0 l&#8217;int\u00e9rieur du vagin (1er degr\u00e9), atteindre la vulve (2e degr\u00e9) ou se projeter vers l&#8217;ext\u00e9rieur de la vulve (3e degr\u00e9).<\/p>\n<p>Il s&#8217;agit d&#8217;une pathologie b\u00e9nigne et courante qui, m\u00eame si elle peut \u00eatre impressionnante et \u00e9ventuellement effrayer la femme, <strong>n&#8217;a pas d&#8217;impact s\u00e9rieux sur sa sant\u00e9.<\/strong><\/p>\n<p>Elle survient principalement chez les femmes multipares \u00e2g\u00e9es ayant des ant\u00e9c\u00e9dents d&#8217;hyst\u00e9rectomie totale et est due \u00e0 un \u00ab rel\u00e2chement \u00bb des tissus pelviens.<br \/>\nOn estime qu&#8217;une femme sur neuf, au cours de sa vie, subira une hyst\u00e9rectomie totale et parmi celles-ci, 10 % pr\u00e9senteront un prolapsus symptomatique du plancher pelvien.<\/p>\n<p>Le fait qu\u2019une femme ait un prolapsus ne signifie pas n\u00e9cessairement qu\u2019elle pr\u00e9sente \u00e9galement des sympt\u00f4mes. Il se peut qu&#8217;elle n&#8217;en soit m\u00eame pas consciente et qu&#8217;elle soit diagnostiqu\u00e9e par hasard lors d&#8217;un examen de routine chez son gyn\u00e9cologue, ou en raison d&#8217;infections urinaires r\u00e9currentes. S&#8217;il s&#8217;agit d&#8217;un prolapsus plus important, la femme peut alors ressentir une sensation de pl\u00e9nitude dans le bas du vagin ou voir quelque chose comme un \u00ab \u0153uf \u00bb sortant du vagin, souvent en fin de journ\u00e9e.<\/p>\n<p>Le traitement d\u00e9pend de l&#8217;inconfort de la femme, de son impact sur sa qualit\u00e9 de vie, du degr\u00e9 de prolapsus et de son \u00e2ge.<br \/>\nIl n\u2019existe pas de traitement pharmaceutique puisque le probl\u00e8me est purement m\u00e9canique, on s\u2019oriente donc vers une solution conservatrice, comme la pose de pessaires vaginaux.<\/p>\n<p>Les pessaires vaginaux sont des anneaux g\u00e9n\u00e9ralement en silicone, qui sont plac\u00e9s au cabinet par voie intravaginale par le gyn\u00e9cologue (aucune sorte d&#8217;anesth\u00e9sie n&#8217;est requise). De cette mani\u00e8re, les organes externalis\u00e9s sont maintenus temporairement en place.<\/p>\n<p>Une solution plus radicale et permanente est bien entendu la restauration chirurgicale du prolapsus.<br \/>\nLa technique de la colporraphie (ant\u00e9rieure, post\u00e9rieure) est la technique classique de restauration du prolapsus des organes pelviens f\u00e9minins. Mais comme la pathologie du prolapsus est due au rel\u00e2chement des tissus, les r\u00e9sultats de cette technique d\u00e9pendent grandement de la qualit\u00e9 des tissus du patient. Plus les tissus sont l\u00e2ches, plus les taux de r\u00e9cidive du prolapsus sont \u00e9lev\u00e9s en postop\u00e9ratoire. On estime que jusqu\u2019\u00e0 50 % des femmes op\u00e9r\u00e9es avec cette technique conna\u00eetront une r\u00e9cidive du prolapsus dans les ann\u00e9es suivantes.<br \/>\nN\u00e9anmoins, elle est largement appliqu\u00e9e dans le monde entier, principalement chez les femmes \u00e2g\u00e9es.<\/p>\n<p>Une autre technique beaucoup plus sp\u00e9cialis\u00e9e et plus r\u00e9cente est la suspension du plancher pelvien avec un treillis synth\u00e9tique en polypropyl\u00e8ne. De cette fa\u00e7on, nous restaurons l&#8217;anatomie en renfor\u00e7ant les parois pelviennes l\u00e2ches avec le maillage, am\u00e9liorant ainsi les taux de r\u00e9cidive postop\u00e9ratoire.<br \/>\nAu d\u00e9but, cette technique \u00e9tait appliqu\u00e9e par chirurgie ouverte, mais aujourd&#8217;hui, avec le d\u00e9veloppement de la technologie, elle est r\u00e9alis\u00e9e presque exclusivement par laparoscopie, par des gyn\u00e9cologues sp\u00e9cialis\u00e9s, avec des temps de r\u00e9cup\u00e9ration plus rapides et de meilleurs r\u00e9sultats esth\u00e9tiques.<br \/>\nOn estime \u00e0 partir d&#8217;\u00e9tudes que le taux de restauration anatomique pendant plus de deux ans apr\u00e8s la chirurgie est de 90 \u00e0 100 %, le taux de satisfaction des femmes est de 70 \u00e0 100 % tandis que le taux de r\u00e9cidive varie de 0 \u00e0 12 %.<br \/>\nLes deux proc\u00e9dures les plus courantes de ce type sont la <strong>promontofixation<\/strong> et la <strong>pectopexie<\/strong>.
