<strong>b\u00e9nigne hormono-d\u00e9pendante<\/strong> (des \u0153strog\u00e8nes naturels de la femme).<br \/>\nIl s\u2019agit d\u2019une pathologie \u00ab mal comprise \u00bb, dont les sympt\u00f4mes et la fr\u00e9quence sont g\u00e9n\u00e9ralement sous-estim\u00e9s.<br \/>\nLe d\u00e9lai moyen entre l&#8217;apparition des sympt\u00f4mes et la premi\u00e8re visite chez le gyn\u00e9cologue est de 4 ans, avec encore 4 ans d&#8217;attente jusqu&#8217;au diagnostic final.<br \/>\nOn estime qu\u2019environ 10 \u00e0 20 % des femmes en \u00e2ge de procr\u00e9er souffrent d\u2019endom\u00e9triose. Toutefois, cela ne veut pas dire que toutes ces femmes pr\u00e9sentent des sympt\u00f4mes.<\/p>\n<h2><strong>Sympt\u00f4mes<\/strong><\/h2>\n<p>Il s&#8217;agit principalement de douleurs pelviennes, de dysm\u00e9norrh\u00e9e (douleurs accrues pendant les r\u00e8gles), de dyspareunie (douleurs lors des rapports sexuels), de dysch\u00e9sie (douleurs lors de la d\u00e9f\u00e9cation), d&#8217;infertilit\u00e9 et dans les cas plus graves d&#8217;h\u00e9maturie (sang dans les urines), de sang dans les selles et plus rarement. h\u00e9moptysie, pneumothorax, etc.<br \/>\nLes sympt\u00f4mes de l&#8217;endom\u00e9triose sont h\u00e9t\u00e9rog\u00e8nes et ne sont pas n\u00e9cessairement li\u00e9s \u00e0 l&#8217;\u00e9tendue des l\u00e9sions. Cela signifie que les femmes pr\u00e9sentant peu de l\u00e9sions peuvent pr\u00e9senter des sympt\u00f4mes graves et vice versa.<br \/>\nParmi toutes les femmes atteintes d\u2019endom\u00e9triose, 60 % souffrent de douleurs pelviennes, d\u2019intensit\u00e9 et de fr\u00e9quence variables, tandis que 30 % ont une fertilit\u00e9 r\u00e9duite.<br \/>\nLa douleur peut aller d&#8217;un inconfort occasionnel et impr\u00e9visible qui s&#8217;att\u00e9nue avec de simples analg\u00e9siques \u00e0 une douleur quotidienne qui affecte s\u00e9rieusement la qualit\u00e9 de vie d&#8217;une femme.<\/p>\n<h2><strong>Diagnostic et tests<\/strong><\/h2>\n<p>Bien qu&#8217;avec une anamn\u00e8se d\u00e9taill\u00e9e et cibl\u00e9e, le gyn\u00e9cologue soup\u00e7onne l&#8217;existence d&#8217;une endom\u00e9triose, une d\u00e9tection par des tests d&#8217;imagerie, comme l&#8217;\u00e9chographie et l&#8217;IRM, est n\u00e9cessaire pour <strong>confirmer<\/strong> le diagnostic. Si m\u00eame avec ces tests, nous ne pouvons pas avoir un diagnostic clair, alors la laparoscopie est recommand\u00e9e, ce qui dissipera tout doute, car c&#8217;est aussi le \u00ab gold standard \u00bb pour le diagnostic final. La laparoscopie, en plus du diagnostic, nous permet d&#8217;\u00e9liminer les \u00e9ventuels foyers d&#8217;endom\u00e9triose que l&#8217;on trouve au m\u00eame moment chirurgical.<br \/>\nSi une endom\u00e9triose intestinale est suspect\u00e9e, un lavement baryt\u00e9 \u00e0 double contraste (examen radiographique de l&#8217;intestin) ou m\u00eame une colonoscopie peuvent \u00eatre n\u00e9cessaires.<\/p>\n<h2><strong>Comment est-ce caus\u00e9 ?<\/strong><\/h2>\n<p>Diverses th\u00e9ories ont \u00e9t\u00e9 formul\u00e9es sur l&#8217;\u00e9tiologie de l&#8217;endom\u00e9triose, l&#8217;une des plus r\u00e9pandues \u00e9tant la th\u00e9orie de l&#8217;implantation, bien que ces derni\u00e8res ann\u00e9es, elle ait \u00e9t\u00e9 contest\u00e9e par d&#8217;autres th\u00e9ories plus complexes.<br \/>\nSelon elle, les cellules de la cavit\u00e9 interne de l&#8217;ut\u00e9rus (endom\u00e8tre), pendant la p\u00e9riode, refluent vers l&#8217;arri\u00e8re et \u00e0 travers les trompes de Fallope de l&#8217;ut\u00e9rus, s&#8217;implantent dans la cavit\u00e9 p\u00e9riton\u00e9ale (int\u00e9rieur de l&#8217;abdomen).<br \/>\nCette implantation provoque une r\u00e9action inflammatoire \u00e0 l\u2019int\u00e9rieur de l\u2019abdomen, qui \u00e0 son tour cr\u00e9e des douleurs et des adh\u00e9rences, entra\u00eenant une infertilit\u00e9 \u00e0 long terme.<\/p>\n<p>Les organes touch\u00e9s sont l&#8217;ut\u00e9rus, les trompes de Fallope, les ovaires<br \/>\n(kystes &#8220;chocolat&#8221;), l&#8217;intestin, l&#8217;appendice, la vessie, les uret\u00e8res, le diaphragme, etc.<\/p>\n<h2><strong>Traitement<\/strong><\/h2>\n<p>Th\u00e9oriquement, \u00e0 l&#8217;\u00e2ge de la m\u00e9nopause, les sympt\u00f4mes de l&#8217;endom\u00e9triose cessent, tout comme les r\u00e8gles de la femme, en raison de l&#8217;arr\u00eat de la production d&#8217;\u0153strog\u00e8nes naturels de la femme.<br \/>\nParfois, cependant, parce que les dommages peuvent \u00eatre importants, la douleur persiste plus longtemps.<br \/>\nChez les femmes en \u00e2ge de procr\u00e9er, le traitement peut \u00eatre conservateur, m\u00e9dicamenteux et\/ou chirurgical.<\/p>\n<p>Le traitement m\u00e9dical est hormonal, par exemple des contraceptifs ou une pr\u00e9paration contenant de la progest\u00e9rone. Mais g\u00e9n\u00e9ralement, cela ne suffit pas, sauf dans les cas l\u00e9gers.<\/p>\n<p>Le traitement chirurgical est d\u00e9sormais r\u00e9alis\u00e9 presque exclusivement par laparoscopie, \u00e9vitant ainsi les hospitalisations de plusieurs jours (1 \u00e0 2 jours) et am\u00e9liorant consid\u00e9rablement la r\u00e9cup\u00e9ration postop\u00e9ratoire.<br \/>\nDans les cas moins s\u00e9v\u00e8res, une excision locale ou une caut\u00e9risation des l\u00e9sions suffit. Dans les cas plus graves o\u00f9 le syst\u00e8me urinaire ou l&#8217;intestin est atteint, des interventions chirurgicales plus \u00e9tendues sont n\u00e9cessaires, telles que l&#8217;ablation d&#8217;une partie de l&#8217;intestin, de la vessie ou de l&#8217;uret\u00e8re. Dans ces cas particuliers, la collaboration d\u2019une \u00e9quipe de gyn\u00e9cologues, d\u2019urologues et de chirurgiens g\u00e9n\u00e9ralistes est indispensable.<\/p>\n<p>Mais dans tous les cas, le traitement est individualis\u00e9 et adapt\u00e9 en fonction des sympt\u00f4mes, de l\u2019\u00e2ge du patient, du d\u00e9sir d\u2019avoir des enfants et de l\u2019\u00e9tendue des
