chirurgicale de l&#8217;ut\u00e9rus d&#8217;une femme.<br \/>\nC&#8217;est la chirurgie gyn\u00e9cologique par excellence et elle est pratiqu\u00e9e pour diverses pathologies f\u00e9minines.<\/p>\n<p>Selon le type d\u2019hyst\u00e9rectomie, on parle de :<\/p>\n<ul>\n<li>totale (ablation du corps de l&#8217;ut\u00e9rus et du col)<\/li>\n<li>subtotale (ablation du corps et pr\u00e9servation du col)<\/li>\n<li>avec les annexes ou sans (si on enl\u00e8ve les ovaires en m\u00eame temps ou non)<\/li>\n<li>radicale (chirurgie plus \u00e9tendue et sp\u00e9cialis\u00e9e qui concerne principalement les tumeurs malignes du col ut\u00e9rin)<\/li>\n<\/ul>\n<p>Ainsi, les types d&#8217;hyst\u00e9rectomie sont adapt\u00e9s \u00e0 chaque pathologie pour laquelle le gyn\u00e9cologue traitant indique une indication chirurgicale.<br \/>\nLes indications peuvent \u00eatre :<\/p>\n<ul>\n<li>saignements vaginaux (ad\u00e9nomyose, fibromes, tumeur maligne)<\/li>\n<li>douleurs pelviennes (ad\u00e9nomyose, fibromes)<\/li>\n<li>prolapsus de l&#8217;ut\u00e9rus (rel\u00e2chement des tissus)<\/li>\n<li>tumeurs malignes de l&#8217;appareil reproducteur (cancer du col de l&#8217;ut\u00e9rus, cancer de l&#8217;ut\u00e9rus, cancer des trompes de Fallope, cancer des ovaires)<\/li>\n<\/ul>\n<p>Apr\u00e8s l&#8217;op\u00e9ration, avec les analg\u00e9siques adapt\u00e9s les premiers jours, la douleur est tout \u00e0 fait g\u00e9rable et ne g\u00eane pas la vie quotidienne du patient. La m\u00e9thode avec laquelle l&#8217;op\u00e9ration a \u00e9t\u00e9 r\u00e9alis\u00e9e y contribue \u00e9galement (par exemple, avec la laparoscopie, la douleur est n\u00e9gligeable par rapport \u00e0 la m\u00e9thode ouverte).<\/p>\n<p>Ce qui inqui\u00e8te et constitue souvent un facteur inhibiteur dans la d\u00e9cision d&#8217;une femme de subir une intervention chirurgicale, c&#8217;est la peur de sa vie sexuelle ult\u00e9rieure.<br \/>\nIl convient de pr\u00e9ciser que l&#8217;hyst\u00e9rectomie en tant qu&#8217;op\u00e9ration n&#8217;affecte pas l&#8217;activit\u00e9 sexuelle, une fois bien s\u00fbr pass\u00e9e la p\u00e9riode postop\u00e9ratoire n\u00e9cessaire de cicatrisation de la plaie interne.<br \/>\nAussi, la pr\u00e9servation ou non du col ne joue aucun r\u00f4le dans l&#8217;activit\u00e9 sexuelle, ni pour elle ni pour son partenaire. Au contraire, la femme lib\u00e9r\u00e9e des probl\u00e8mes qui la tourmentaient avant l\u2019op\u00e9ration, peut d\u00e9sormais mener une vie <strong>sexuelle normale<\/strong>.<\/p>\n<h2><strong>Types d&#8217;hyst\u00e9rectomie<\/strong><\/h2>\n<p>L&#8217;hyst\u00e9rectomie peut \u00eatre r\u00e9alis\u00e9e selon trois m\u00e9thodes diff\u00e9rentes, sous anesth\u00e9sie g\u00e9n\u00e9rale.<\/p>\n<ul>\n<li>La m\u00e9thode classique \u00ab ouverte \u00bb o\u00f9 l&#8217;on op\u00e8re avec une incision transversale (comme une c\u00e9sarienne) juste au-dessus de l&#8217;os pubien.<\/li>\n<li>La m\u00e9thode vaginale, o\u00f9 l&#8217;ablation de l&#8217;ut\u00e9rus se fait enti\u00e8rement par la voie naturelle (vaginale), sans incision \u00e9vidente.<\/li>\n<li>Laparoscopie, avec laquelle l&#8217;op\u00e9ration est r\u00e9alis\u00e9e \u00e0 travers quatre trous (5 millim\u00e8tres chacun) dans l&#8217;abdomen.<\/li>\n<\/ul>\n<p>Chaque m\u00e9thode est appliqu\u00e9e en fonction de la pathologie sous-jacente en consultation avec les souhaits du patient.<br \/>\nEn g\u00e9n\u00e9ral, lorsque l&#8217;ut\u00e9rus a de tr\u00e8s grandes dimensions, on applique la m\u00e9thode ouverte, tandis que dans la mesure du possible, on pr\u00e9f\u00e8re la m\u00e9thode laparoscopique, car elle offre une r\u00e9cup\u00e9ration presque indolore et tr\u00e8s rapide avec de tr\u00e8s bons r\u00e9sultats esth\u00e9tiques. De plus, avec l&#8217;hyst\u00e9rectomie laparoscopique, nous pouvons voir et examiner tout l&#8217;abdomen et r\u00e9aliser une hyst\u00e9rectomie totale (ablation du col et de l&#8217;ut\u00e9rus), une hyst\u00e9rectomie partielle (hyst\u00e9rectomie subtotale, c&#8217;est-\u00e0-dire pr\u00e9servation du col) ainsi qu&#8217;intervenir dans d&#8217;autres parties de l&#8217;abdomen si n\u00e9cessaire, comme les trompes de Fallope et les ovaires ou encore l&#8217;appendice.<br \/>\nL&#8217;hyst\u00e9rectomie vaginale donne d&#8217;excellents r\u00e9sultats postop\u00e9ratoires, mais est en retard sur le fait qu&#8217;avec cette m\u00e9thode, nous ne contr\u00f4lons pas l&#8217;abdomen pendant l&#8217;op\u00e9ration et qu&#8217;en outre, elle n&#8217;est pas r\u00e9alis\u00e9e en cas de tumeur maligne.<\/p>\n<h2><strong>Sang apr\u00e8s hyst\u00e9rectomie<\/strong><\/h2>\n<p>Apr\u00e8s une hyst\u00e9rectomie, quelle que soit la m\u00e9thode, si le col est \u00e9galement retir\u00e9 (hyst\u00e9rectomie totale), le vagin est sutur\u00e9 avec des fils qui se r\u00e9sorbent d&#8217;eux-m\u00eames au bout de plusieurs jours. Pendant ce temps, la cicatrisation interne a eu lieu et il n\u2019y a aucun risque d\u2019ouverture de la plaie.<br \/>\nEn g\u00e9n\u00e9ral, nous recommandons l\u2019abstinence de tout rapport sexuel pendant 6 \u00e0 8 semaines apr\u00e8s la chirurgie.<br \/>\nDans certains cas, cependant, en raison d&#8217;une infection locale ou d&#8217;un rapport sexuel pr\u00e9matur\u00e9, les points de suture peuvent s&#8217;ouvrir et des saignements vaginaux peuvent survenir. Elle est g\u00e9n\u00e9ralement de peu d&#8217;intensit\u00e9, mais elle n\u00e9cessite dans tous les cas une \u00e9valuation par le gyn\u00e9cologue traitant.<br \/>\nDans les premiers jours apr\u00e8s l&#8217;op\u00e9ration, cependant, la pr\u00e9sence de sang est normale puisque ce sont des d\u00e9chets qui sont expuls\u00e9s du corps et il n&#8217;y a aucune raison de s&#8217;inqui\u00e9ter.<\/p>\n<h2><strong>Gonflement apr\u00e8s une hyst\u00e9rectomie<\/strong><\/h2>\n<p>Un gonflement apr\u00e8s une hyst\u00e9rectomie peut survenir et durer quelques jours. Cela est g\u00e9n\u00e9ralement d\u00fb au fait que les intestins, en raison de l&#8217;op\u00e9ration, n&#8217;ont pas retrouv\u00e9 leur fonction normale, ce qui entra\u00eene une accumulation de gaz et de mati\u00e8res f\u00e9cales \u00e0 l&#8217;int\u00e9rieur et provoque une sensation de lourdeur et de gonflement. Elle est plus fr\u00e9quente en cas de chirurgie ouverte et beaucoup moins voire pas du tout en cas d&#8217;hyst\u00e9rectomie laparoscopique ou d&#8217;hyst\u00e9rectomie vaginale. Habituellement, apr\u00e8s 2-3 jours, l&#8217;intestin commence \u00e0 \u00ab fonctionner \u00bb et le gonflement dispara\u00eet.<br \/>\nLa m\u00e9thode laparoscopique et vaginale contribue \u00e0 la r\u00e9cup\u00e9ration rapide du gonflement, puisque la patiente est mobilis\u00e9e beaucoup plus rapidement apr\u00e8s l&#8217;op\u00e9ration en raison de la r\u00e9duction de la douleur postop\u00e9ratoire. Si l&#8217;intestin ne fonctionne pas au bout de 3 jours, il existe alors un risque d&#8217;il\u00e9us et la situation doit \u00eatre \u00e9valu\u00e9e par le gyn\u00e9cologue traitant.<br \/>\nDans certains cas d&#8217;hyst\u00e9rectomie laparoscopique, un gonflement abdominal et des douleurs \u00e0 l&#8217;\u00e9paule peuvent survenir, dus au dioxyde de carbone pr\u00e9sent dans l&#8217;abdomen apr\u00e8s l&#8217;op\u00e9ration. C\u2019est un sympt\u00f4me innocent qui dispara\u00eet apr\u00e8s 24 \u00e0 48 heures, lorsque le dioxyde de carbone est absorb\u00e9 par l\u2019organisme.<\/p>\n<p><img decoding=\"async\"
