spontan\u00e9es de grossesse et constituent la complication la plus courante de la grossesse au cours du premier trimestre.<\/p>\n<h2><strong>Quand cela peut-il arriver ?<\/strong><\/h2>\n<p>Sa fr\u00e9quence jusqu&#8217;\u00e0 20 semaines de la premi\u00e8re grossesse est estim\u00e9e entre 8 et 20 %, alors qu&#8217;elle diminue jusqu&#8217;\u00e0 5 % chez les femmes ayant d\u00e9j\u00e0 un enfant.<br \/>\nIl diminue \u00e9galement de fa\u00e7on spectaculaire apr\u00e8s 15 semaines (0,6 %).<\/p>\n<p>Les facteurs de risque de fausses couches sont principalement l&#8217;\u00e2ge avanc\u00e9 de la femme, les ant\u00e9c\u00e9dents, le tabagisme, l&#8217;alcool.<br \/>\n\u00c0 titre indicatif, le risque pour une femme de 35 ans peut atteindre 20 %, pour une femme de 40 ans 40 %, alors qu&#8217;il peut atteindre jusqu&#8217;\u00e0 80 % chez une femme de 45 ans.<\/p>\n<p>Jusqu&#8217;\u00e0 50 % de ces fausses couches sont dues \u00e0 des anomalies chromosomiques du f\u0153tus, qui emp\u00eachent la poursuite de la grossesse.<br \/>\nLe reste est d\u00fb \u00e0 des anomalies anatomiques de l&#8217;ut\u00e9rus (principalement la pr\u00e9sence d&#8217;un diaphragme), \u00e0 \u200b\u200bune infection du f\u0153tus (par exemple Listeria), \u00e0 certains troubles des facteurs de coagulation (thrombophilie apr\u00e8s 10 semaines), \u00e0 une endocrinopathie non diagnostiqu\u00e9e de la m\u00e8re (par maladie thyroidienne), ou enfin \u00e0 des raisons inconnues.<\/p>\n<h2><strong>Sympt\u00f4mes<\/strong><\/h2>\n<p>Une fausse couche peut ne pr\u00e9senter aucun sympt\u00f4me ou se manifester par des saignements vaginaux et des douleurs abdominales basses.<\/p>\n<p>Le diagnostic se fait par \u00e9chographie et en laboratoire, par mesure de la b\u00eata-gonadotrophine chorionique, la principale hormone de grossesse.<br \/>\nDans ces cas, nous avons une interruption de la fonction cardiaque du f\u0153tus, qui s&#8217;accompagne d&#8217;une baisse des taux de gonadotrophines chorioniques \u03b2.<br \/>\nParfois, lorsque l&#8217;on est en tout d\u00e9but de grossesse, le diagnostic diff\u00e9rentiel avec une grossesse extra-ut\u00e9rine peut \u00eatre difficile, car le f\u0153tus ne peut pas \u00eatre visualis\u00e9 par \u00e9chographie. Dans ces cas-l\u00e0, la surveillance constitue g\u00e9n\u00e9ralement la solution.<\/p>\n<p>En raison du saignement, la masse est expuls\u00e9e totalement ou partiellement, un curetage est donc n\u00e9cessaire sous anesth\u00e9sie g\u00e9n\u00e9rale pour \u00ab nettoyer \u00bb l&#8217;int\u00e9rieur de l&#8217;ut\u00e9rus et arr\u00eater le saignement, qui peut \u00eatre grave. Il s\u2019agit d\u2019une intervention chirurgicale courante qui n\u00e9cessite une hospitalisation d\u2019une journ\u00e9e et ne devrait pas effrayer les patients.<br \/>\nDans certains cas
